Membership Application

Anyone wishing to become a Supporting Member of Georgia Kayak Fishing, Inc. must submit a completed membership form and make a payment using the following options. Once registration and payment are received you will receive your benefits through 2011.

TopSubmitting a form

A. Fill out the online form below. Once it filled out completely, submit it by hitting the “Submit” button. You must hit the “Submit” button, even if you’re paying with PayPal. Once it is received by the Board of Directors, you will receive an email confirming your membership and selections.

B. Fill out the form manually. Print this page. Once the page is printed, you can fill out the form by hand and mail it to our treasurer at the follow address:

Once it is received by the Board of Directors, you will receive an email confirming your membership and selections.

TopSubmitting your payment

A. Submit payment online using PayPal (PayPal Account is NOT NEEDED to pay via a credit card using PayPal). Below the online form there is a PayPal drop down menu. Select the membership level you wish to purchase. Click the “Add to Cart” Button below the drop down menu and follow the PayPal instructions. Be sure to still hit the “Submit” button on the form if you’re paying with PayPal.

B. Submit payment via Cash, Check or Money Order. To submit your payment this way, please make checks payable to GKF, Inc. and mail your payment to:

By submitting this document, becoming a member, or participating in any GKF, Inc. events, the above named person,

1. Agrees to follow the by-laws of the GKF, Inc.

2. Acknowledges and fully understands that he or she will be engaging in activities that involve risk of serious injury including permanent disability and death which might result not only from his/her actions, inactions and negligence but the actions of, inactions and negligence of others or the condition of equipment used. Furthermore, there may be risks not known to us or not reasonably foreseeable at this time.

3. Assumes the foregoing risks and accepts personal responsibility for the damages following such injury, permanent disability or death.
